Please provide the following information in relation to hurricane swings, rotating swing, or similar spinning swing equipment.

We request this information from 1st January 2016 to date

1. A list of all sites where such swings have been installed

2. Records of complaints received relating to these swings, including the nature of the complaint and the date received

3. Records of any reported accidents, injuries, or incidents involving these swings

4. Details of any investigations carried out following complaints, incidents, or accident involving these swings

5. Copies of risk assessments, safety reviews, inspection reports or safety audits relating to these swings

6. Any internal safety guidance, policies or documented health and safety considerations relating specifically to the installation, maintenance or operation of these swings

7. Details of any of these swings being decommissioned including, location of swing, date of removal, reasons for removal.
