National Entitlement Card - How to Apply

Application forms are available by telephoning 0300 123 4565, emailing accessmoray@moray.gov.uk, or by picking up a form at your local access point or library.  Young people in Primary 7 will receive a letter through their primary school which includes a consent form so that you can apply for a card before starting secondary school.

Application forms should be returned in person to your local access point or library.  You will need to take with you:-

- Colour passport style photograph (bookmark to below)
- Proof of your name and age (bookmark to below)
- Proof of your address (bookmark to below)
- Proof of photo (bookmark to below)
- Proof of your disability (if applying on the grounds of disability) (bookmark to below)
- Proof of your volunteer status (if applying as a 19-25 year old volunteer working 30 or more hours per week) (bookmark to below)

Once your application has been processed a National Entitlement Card will be requested and should arrive by post to your home address within 2 weeks.

Photograph
Please bring one passport type colour photograph.  The photograph should be a close-up of your head.  Please do not wear a hat or head covering (unless for religious reasons) or have your hood up or wear dark glasses.

Proof of name and age
An original (not photocopy) of one of the following is acceptable:-
- Photo driving licence
- Passport (current)
- Birth certificate – or an adoption/marriage certificate if your name has changed
- An official government document such as an asylum registration, immigration status, or a UK Border Agency travel document
- PASS Hologrammed Card

Proof of address
An original (not photocopy) of one of the following is acceptable:-
- Home utility bill (not older than 3 months and not a mobile phone bill)
- Council tax invoice (current year)
- Moray Council or housing association rent card / tenancy agreement (current year)
- Bank or building society statement (not older than 3 months)
- Credit card statement (not older than 3 months)
- Television licence (current year)
- Mortgage statement (current year)
- HMRC tax office letter (current year)
- Department for Work & Pensions benefits letter or Tax Credits letter (current year)
- Occupational pension letter (current year)
- Letter from Care or Residential Home

Proof of photo
An original (not photocopy) of one of the following is acceptable:-
- Photo Driving Licence
- Passport (current)
- PASS Hologrammed Card

Proof of disability (if applying on the grounds of disability)

Proof of volunteer status (if applying as a 19-25 year old volunteer working 30 or more hours per week)
An original (not photocopy) of one of the following is acceptable:-
- Project Scotland membership card
- CSV Scotland registration letter
