1.  Does your organisation provide traditional in-house Day Services?  (owned and operated by your organisation)

If yes, do you have criteria applied for individuals accessing these services? If you do have a criteria could you possibly share it with me.  

2.  In lieu of providing in-house Day Services, what do you support people to access?

3.  Does your organisation provide subsidised Social Care transport? i.e. transport which is paid for by the organisation?

4.  If you have a Social Care Transport Policy – could you please share it with us.
