1.  Over the last five academic years, the total number (and details of these incidents) of incidents where a pupil has caused injury to (a) a teacher or staff member, or (b) another pupil.

2.  Number of these incidents that were (a) recorded in an official log or record book, (b) had attendance from a first aider, or (c) where the Scottish Ambulance Service was called.

3.  For each incident involving a pupil causing injury, please provide information on the resulting disciplinary action, such as (a) detention, (b) suspension, (c) exclusion, or (d) no action taken.
