In 2022 the East Community Care Team was part of a pilot with Partners4Change to adopt the 3 Conversations Model. Other Social Work teams were also part of this, Learning Disability Team, and the Hospital Discharge Team. How much was spent on this consultancy work and resulting outcomes in total (e.g. new paperwork, weekly reflective sessions with Partners4Change).
