FOI Request - Time-Off for Religious Observance

I am writing to you to investigate current practices across Local Authorities in relation to time off for religious observance.
To that end, I am submitting an official Freedom of Information request in writing to request information on:
1.  the current policies and protocols your local authority has in place for allowing teachers to request time off for religious observance
2.  the protocols used for recording such requests
3.  number of applications received in the last two years, and percentages of acceptance/rejection
4.  justification for instances where requests have been rejected
5.  any equality monitoring carried out in relation to the protected characteristic of applicants, and rates of acceptance/rejection
