FOI Request - Care Packages

In accordance with the Freedom of Information Act 2002, I would be grateful if you could comply with the following request:
1. How many people in your IJB area who have been assessed as having (a) critical, (b) substantial, (c) moderate, or (d) low care needs are currently (i) awaiting a care package, or (ii) awaiting an increase to an existing care package.
2. What is the longest that any person currently awaiting a critical care package has been waiting, and how many people have waited over (a) 3 months and (b) 6 months?
3. How many people are currently in care homes awaiting the provision of a package of care at home?
4. How many people in the last 12 months have been assessed as having substantial care needs, and of those how many have been provided with a care package.
5. Please provide a copy of any reports or other documents that discuss the issue of critical care packages not being fulfilled.
