1. Name and address of organisations, companies or charities which are contracted to provide adult social care
2. Name and address of service/centres where adult social care is provided
3. Amount of money paid to each organisation, company or charity in line with their agreed contract for adult social care
4. Date contract was commenced, length of contract and confirmation as to whether there is an automatic renewal to the contract
5. Date of renewal of contract
