FOI Request - Child Practice Reviews

This is a request for information under the Freedom of Information Act 2000. My request is directed at the Local Safeguarding Partnership / Board.

I would like to request the following information:

1) Between 1st January 2021 and 31st December 2021, how many Child Safeguarding Practice Reviews or Child Practice Reviews were initiated?

1a) Of these reviews, how many were initiated because of the death of a child, and in what year did that death occur?

2) Can you please provide the same data requested in questions 1 and 1a for the following time periods:

Between 1st January 2017 and 31st December 2017
Between 1st January 2018 and 31st December 2018
Between 1st January 2019 and 31st December 2019
Between 1st January 2020 and 31st December 2020
