FOI Request - Workplace Injury

Please provide me the following information with regards to the way in which the council records workplace incidents in which a member of staff is injured.
1. Regarding this system, please tells me – broken down month-by-month for the last 24 full calendar months – how many such incidents were recorded across all council premises and at which locations were these recorded?
2. Additionally, please provide a separate monthly breakdown detailing the level of injury – minor, severe, etc. (or whichever categories are used by your reporting system) and what these levels approximate to (e.g. severe means a broken bone) .
