FOI Request - Funded Childcare Application Forms

I would like to request the following information:

Copies of the application forms related to funded childcare in your local authority.

I would request that the information include:

1.       The application form (or the most relevant documentation) and supporting explanatory documentation that private and third sector providers and childminders in your local authority use to request funding for providing the funded childcare hours (both the current and expanding to 1140 hours entitlements if these are different documents).

2.       The application form (or the most relevant documentation) and supporting explanatory documentation that parents and carers in your local authority use to inform the council how they want to use their funded entitlement for childcare.
