Throughcare and Aftercare
The target group is ages 14-21 (extended to 25 if in further education or at university).
Throughcare is the on-going preparation and support for young people who are currently looked after and may soon be making the transition to live more independently. Aftercare is the continuing support, planning and guidance for a young person who has left care or a supported living environment.
The service provides support to access employment, education and training, including mock interviews, searching for jobs online and providing advice and support for making job-related phone calls. The service would support young people to attend careers appointments and DWP appointments as appropriate. There is partnership working with employability agencies where the young person allows it. The focus is on helping the young person make their own informed choices
