Community Care Assessment

We will organise a care assessment to understand your care and support needs and work with you to find the best methods to provide care and support. There is no charge for a care assessment.

How to arrange a care assessment

You or someone on your behalf can contact us to discuss your support needs and arrange an assessment, if required. You may be referred by a doctor or social care professional.

If you are an unpaid carer you can request an assessment to discuss your situation and the support that would help you to continue in your caring role.

What happens next

A community care officer or social worker will arrange to visit you to discuss and understand your needs and determine the services and equipment that will best support you. It may be helpful to have a carer or family member present for the meeting.
Your situation will be evaluated based on our eligibility criteria.

Your care plan

A care plan will be agreed after your assessment. It sets out the support that you need:

• What help you will get and who will provide it
• When and how often you will get help
• When help will start and timings of reviews
• Paying for care
• What to do if you are unhappy with the service.
